1. Are your pads approved in all states?
Our pads security features meet and exceed all CMS guidelines requested by the federal government for Medicaid prescriptions as of April 1, 2008.
Some states require special requirements exceeding CMS requirements. They are:
- California
- Florida
- Indiana
- Kentucky
- Maine
- New Jersey
- West Virginia
Our paper may not meet all the features of those above state’s requirements.
There are several other states that, while they do not require special security features exceeding CMS guidelines, do request a “special format” to be used in the printing.
They are:
- Louisiana
- Massachusetts
- Mississippi
- Pennsylvania
- Texas
If you are ordering for one of those states, please fax a sample of what you would like to have printed, along with your contact information, to us at 314-802-3512. One of our staff will contact you and discuss your needs over the phone.
All practitioners must obtain their prescription forms from the New York State Department of Health.
3. How do I order multiple pads?
You can save each pad you order as an individual project for future reference. You can add multiple pads to your cart as needed.

For pads with consecutive numbering, add $2.00 per pad.
Our Maximum stock is our top of the line one part stock, with the most security features and an attractive preprinted border in both landscape and portrait formats. Our Minimum one and two part stock is slightly less expensive, and offers many of the secure features of our Maximum stock without the preprinted border.
5. What if my desired format isn’t among your templates?
Please fax a desired layout to our staff at 314-802-3512, along with your contact information, and they will assist you with your needs. A one-time setup fee may apply to those custom orders not placed through our website.
6. What is your shipping schedule?
Our normal shipping schedule is 3-5 business days from the time we receive your order. For rush orders, please request expediting from the drop down menu on the order screen.
7. How long will it take my order to arrive?
The map shows a general representation of UPS Ground transit times. All orders shipped from Maryland Heights, MO 63043.
